Form Field Settings
These are the default settings:
Hopefully fairly simple to use. You can select to make it a required field, change when the field displays and edit the labels.
You can also drag and drop to change the order and delete the field.
Clicking the Add Field button brings up a modal which lists all the unused fields:
Click on the field you want to and it will display at the bottom of the fields table.
